Anyone can file a complaint against North Fork Community Paramedicine without retaliation.

Complaint forms can be obtained by:

Complaints can be filed anonymously, and all complaints will be investigated equally. However, we will only be able to follow up with you if we have your name and contact information.
Anyone can file a complaint with Colorado Department of Health and Environment concerning:

• Quality of care.
• Patient/resident rights.
• Building and equipment safety.
